Case # 14                                       <<Back    Next>>

63 year old female with invasive lobular carcinoma in the contralateral breast
Sagittal T2W FS Axial T1W
Round mass with smooth borders, mildly increased T2W signal, high T2W FS signal, intermediate T1W signal (adjacent to cyst)
Enhancement curve
Prompt enhancement in the first phase with minimal progression in the second phase, then minimal dropout in the third phase
Subtraction

First post-enhancement phase

Third post-enhancement phase

Ultrasound
Solid, hypoechoic mass with minute echogenic calcifications, adjacent cyst
Diagnosis: Fibroadenoma
Fibroadenomas can have a variety of enhancement patterns. The literature reports enhancement ranging from none to mimicking invasive carcinoma. Morphologically on MRI, nonenhancing internal septations and macrolobulation are the most helpful in suggesting the diagnosis of fibroadenoma.
