 46 year old female with chronic mastitis

MRI: In the subareolar region and extending to the lower inner and lower outer quadrants of the breast, there are multiple cystic complex fluid-containing structures.  These structures have the appearance of dilated, tortuous ducts with hemorrhagic or proteinaceous contents.

With contrast injection, there is little enhancement of the region surrounding the cystic structures.
Diagnosis: Abscess, surgically resected
The relative lack of enhancement was felt to be unusual. Papillomatosis was included in the differential. The paucity of enhancement could have been due to the chronic and partially treated nature of the disease.
